Please mail Membership Form and fees before the first attending playday to

Compete at one or compete at all, you must be a member. 

Day-Of Entry Form (2 per sheet)

Please print one entry form per contestant per playday.

Turn these into the office the day of with correct money. Must have new entry forms for each playday.
